Authorization of School Personnel to Administer Medications
This provides our staff to administer medication to your students.

Traumatic Head Injury (Concussion) Policy

To ensure the safety and well-being of students by establishing procedures for the prevention, identification, and management of traumatic head injuries, including concussions, in accordance with Utah laws and best practices.

Medication Policy

To ensure the safe administration of medication to students during school hours while promoting student health and wellness.

Epinephrin Auto-Injector Policy

To ensure the safety and well-being of students, staff, and visitors by providing immediate access to epinephrine auto-injectors (EAIs) for individuals experiencing anaphylaxis on school premises.

MMA Illness Policy
To promote a safe and healthy learning environment by reducing the spread of illness and providing clear guidelines for when students should stay home, when they may return, and when they must be sent home from school.

Mental Health Screening
Mental health screening evaluates the possible presence of issues such as anxiety, depression, and suicidality. It is not a diagnostic tool, but it helps identify students who may need support or services. Screenings will only occur after caregivers give written consent, actively opting their child into the program or event. This screening is conducted in partnership with the Utah School Mental Health Collaborative at the University of Utah.

Diabetes Management Plan
A school diabetes management plan outlines how schools support students with diabetes to learn and participate fully, ensuring their safety and well-being.

Seizure Action Plan
A seizure action plan is a detailed document that provides guidance on how to manage and respond to seizures, especially in emergency situations. 

Allergy and Anaphalaxis
An allergy and anaphylaxis form is a crucial document for students with allergies or a history of anaphylaxis. It outlines the specific allergic reactions a student has, symptoms to watch for, and emergency procedures to follow, including medication instructions if applicable. 

Asthma Action Plan
An asthma action plan is a personalized guide that helps students with asthma manage their condition. It outlines how to treat asthma daily, what to do when symptoms worsen, and when to seek emergency care.
